How MAPS Works
MAPS works by offering a simple, standardized way to prepare people to participate in their care—and to document that participation for providers, insurers, and policy teams.
It starts with a set of tools: the Healthcare Proficiency Challenge, Readiness Assessment, Waiver, Completion Survey, and Certification. These are easy-to-use, paper-based or digital forms that walk patients through the process of learning, acting, and proving they’re engaged.
These tools are licensed to healthcare professionals and systems, who can use them with patients at any point in their care—especially at diagnosis.
Each completed step creates measurable, trackable documentation—giving the healthcare system a clearer picture of who’s engaged, who needs help, and how to report it.

We are Built for Patients. Designed for Systems.
From curriculum to implementation, our resources teach patients how to manage their care—and help systems support them every step of the way.
Empowers patients to understand their care, ask informed questions, and follow through—on their own terms and time.
From our underserved to rural communities, our kits are tailored for accessibility, inclusion & relevance.

Our signature tools measure how well prepared patients are to manage care that will empower providers with actionable insight.
We work behind the scenes to help organizations integrate Patient Better smoothly—no tech overhaul required.
